Introduction to the Montana Department of Labor & Industry Self-Insurance Workers’ Compensation Program & Application
Self-insurance applications are available in either Microsoft Excel or Portable Document Format (pdf).
1) The Microsoft Excel format - Please fill in your application information in the colored cells of the Microsoft Excel form. The totals on page 2 of the application will calculate automatically (accidents by year, claims by year, three-year average, unpaid liability, and cash pay-outs). Please enter dates as mm/dd/yyyy (month, day, year).
2) PDF - This has been designed for you to print out on your local printer, so you can then fill in the appropriate information by typewriter or pen.
We will continue to send you an electronic reminder at least 30 days prior to when your renewal packet is due to the Department. Please note that we require an actuary report to be submitted with your application. The report is to be completed by a qualified actuary as defined by the American Academy of Actuaries. It will include a reserve analysis to include all self-insured periods in Montana through the most recently closed calendar year with results summarized at the appropriate corresponding confidence level(s). For renewals, an electronically submitted application is preferred and acceptable if it contains a scanned copy of the authorized signature. PDF format is acceptable for financial statements and excess insurance. These changes and improvements are intended to provide better customer service. We welcome your comments and suggestions.
